This is Scholastics Transition Math program.  It was created using Ultra ISO and can be mounted to a virtual drive using Ultra ISO (or your fav program) or burned to disc. 

I used it to test my kids between the grades to see if we had any weaknesses and also had them play it during summer between school years to keep them brushed up in a fun way.
